Hope I'm in the right area.
Sometimes I see tears that just won't disappear no matter how much I smooth them. So I use this technique.
In terrain tools go to the second one from the left, the one with the cross next to the mountain. Shift click on the tear. This will give you a height reading. Then using that height paint over the tear and slightly beyond so you are flattening the tear. Then smooth the area slightly and the tear should disappear for good.
