Birdie, I see that you are using 3C.  I think the frame rate patch is only for RTSM and possibly MS so there is no reason for you to turn it on.

Also I don't think shadows are fixed yet.  There is a lot of shimmering and very noticeable for the shadow of the avatar.

RTSM - Pro

Yes, you are right. I should have posted this somewhere else as it's probably not related to this specific patch but more to pause render in general. This can be moved if possible. Thanks for sorting my brain! 👉

If I don't use RTS or SLi do I need this patch?

This patch increases the framerate(1.3-1.4 times) regardless of swing types.

You might have noticed that other file(s) in the (14306) patch such as mainData and Assembly-CSharp.dll have in fact changed to a date of just a few days ago and are slightly different in size.  It therefore follows that the files you mention are the same identical files as per the previous (14305) patch and did not require changing/updating.  No need to be be concerned as the patch does what it says on the tin.

Intel i5 4690K (OC to 4.4GHz), Arctic i32 CPU cooler, 16GB DDR3 RAM @2400MHz, Nvidia Inno 3D iChill 1060 (6GB)
