I get these occasionally but don't know exactly what causes them.

This usually happens on an outside surrounding spline.

Sometimes it resolves after adding/deleting/moving control points around or just redrawing the spline.   I just try random things until it works but it can take  awhile.

Does anyone know what actually causes this?   It would be nice to know how to avoid or specifically fix this.

I think it seems to be too sharp of an angle on a lower resolution spline. If you go to debug mode, cycle through the splines until it highlights and that’s the one giving you trouble. Look for sharp turns. At least that’s my experience.

Easiest way to detect problem area is to change viewport to "Shaded wireframe". Typically the mesh error will cast off from a single point in a triangular shape.

As Stingereye noted, almost always just an acute angle. Adding points or "softening/rounding" the angle by moving the point and recalculating works majority of the time.
